Preparing Co-Creation Sessions Across Europe

Over the past months, partners in Spain, Denmark, and Slovenia have been actively working to identify and engage key local stakeholders:  children and adolescents with complex chronic conditions, their family caregivers, healthcare professionals, hospital administrators, and patient advocates.

This work sets the stage for the first round of co-creation sessions, planned for October and November 2025.

The upcoming workshops will bring together lived experience and professional expertise to refine the HOME-C2P2 care coordination model and collaboratively design a meaningful evaluation framework. By centering the voices of families and frontline professionals, these sessions will help shape the digital tools and care practices that support home-based paediatric care.
