We’re excited to announce the launch of the European project HOME-C2P2 (Clinical Command Center to coordinate efforts with primary healthcare), led by Dr. Sílvia Ricart from Fundació de Recerca Sant Joan de Déu.

🧒🏼 The project aims to enhance home care for children with complex chronic conditions or palliative needs by improving coordination between primary and specialized care, using technology tailored to their needs.

🌍 With 6 partners from 5 European countries, HOME-C2P2 will run for 3 years and is funded under the European Partnership on Transforming Health and Care Systems (THCS), part of the EU’s Horizon Europe programme (Grant Agreement No. 101095654).
