Our project at THCS Annual Conference 2025 News 2 March, 2026 Our HOME-C2P2 project was presented in the THCS Annual Conference 2025, as part of the pre-congress of the European Public Health Congress that was held on 11th November in Helsinki. Dr. Silvia Ricart Campos participated in a round table about primary care (Session 2: Strengthening Primary Care). It was an interesting exchange of pilot experiences aimed at transforming the European healthcare system to address the challenge of chronicity and sustainability. Co-creation begins in the HOME-C2P2 Project! News 17 November, 2025 In the months of November and December 2025, the HOME-C2P2 project enters an exciting new phase: co-creation sessions are launching across three leading European pediatric institutions: Hospital Sant Joan de Déu with IDIAP Jordi Gol (Barcelona, Spain) Region Hovedstaden (Copenhagen, Denmark) University Medical Centre Ljubljana (Slovenia) Together with families, pediatric patients, and healthcare professionals, we are working to refine the HOME-C2P2 model — a coordinated home care approach for children with complex chronic conditions or palliative needs. These sessions will explore home-care monitoring journeys, identify systemic gaps, and surface opportunities for improvement — all grounded in the lived experiences of those who give and receive care. Developed by VOH.CoLAB and Erasmus University Medical Center, in collaboration with all consortium partners, our research protocols and facilitation tools are designed to give voice to those who matter most: children, caregivers, and frontline teams. Preparing Co-Creation Sessions Across Europe
Preparing Co-Creation Sessions Across Europe News 31 July, 2025 Over the past months, partners in Spain, Denmark, and Slovenia have been actively working to identify and engage key local stakeholders: children and adolescents with complex chronic conditions, their family caregivers, healthcare professionals, hospital administrators, and patient advocates. This work sets the stage for the first round of co-creation sessions, planned for October and November 2025. The upcoming workshops will bring together lived experience and professional expertise to refine the HOME-C2P2 care coordination model and collaboratively design a meaningful evaluation framework. By centering the voices of families and frontline professionals, these sessions will help shape the digital tools and care practices that support home-based paediatric care.
