Co-creation begins in the HOME-C2P2 Project!

In the months of November and December 2025, the HOME-C2P2 project enters an exciting new phase: co-creation sessions are launching across three leading European pediatric institutions:

  • Hospital Sant Joan de Déu with IDIAP Jordi Gol (Barcelona, Spain)
  • Region Hovedstaden (Copenhagen, Denmark)
  • University Medical Centre Ljubljana (Slovenia)

Together with families, pediatric patients, and healthcare professionals, we are working to refine the HOME-C2P2 model — a coordinated home care approach for children with complex chronic conditions or palliative needs.

These sessions will explore home-care monitoring journeys, identify systemic gaps, and surface opportunities for improvement — all grounded in the lived experiences of those who give and receive care.

Developed by VOH.CoLAB and Erasmus University Medical Center, in collaboration with all consortium partners, our research protocols and facilitation tools are designed to give voice to those who matter most: children, caregivers, and frontline teams.
