HOME-C2P2 is a European initiative to enhance home-based care and quality of life for children with complex chronic or palliative conditions.

Mission and Objectives

The mission of the HOME-C2P2 project is to improve the quality, accessibility, and coordination of home-based care for children with complex chronic conditions and/or palliative needs across Europe.

By integrating home monitoring technologies, coordinated care plans, and primary-specialist collaboration, the project aims to reduce hospital visits and empower families.

Children with complex chronic or palliative care needs face frequent hospitalizations and fragmented care. HOME-C2P2 introduces a shared, personalised care model supported by home-monitoring devices and a digital coordination platform.

HOME-C2P2 objectives reflect a strong commitment to personalised prevention, participatory design, and digital transformation in paediatric palliative and complex care.

1. Facilitate Implementation Across Europe

Identify the clinical, technological, and systemic factors that would enable the implementation of the HOME-C2P2 model in different European countries, acknowledging the diversity of health systems and technological contexts.

2. Co-design with End Users

Engage healthcare professionals and family caregivers in the co-design of both the care model and the supporting technology to ensure it meets their needs and is usable in real-world settings.

3. Test Feasibility and Acceptance

Assess the feasibility, logistics, and acceptability of the HOME-C2P2 model through multi-stakeholder implementation in several European countries (Spain, Denmark, Netherlands, and Slovenia).

4. Promote Integration into Health Systems

Encourage the integration of the model and its digital tools into routine care across primary and specialist levels, using real-time monitoring and shared care plans to improve health outcomes.

The European Partnership on transforming health and care systems (THCS) is a Cofund action under the Horizon Europe Programme  designed to support coordinated national and regional research and innovation programmes along with capacity building, networking, dissemination and other key activities to support health and care systems transformation.

HOME-C2P2 is funded under the THCS Joint Transnational Call 2024 – Innovate to Prevent: Personalised Prevention in Health and Care Services (Grant No. 101095654).
