HOME-C2P2 is a European initiative to enhance home-based care and quality of life for children with complex chronic or palliative conditions.
The mission of the HOME-C2P2 project is to improve the quality, accessibility, and coordination of home-based care for children with complex chronic conditions and/or palliative needs across Europe.
By integrating home monitoring technologies, coordinated care plans, and primary-specialist collaboration, the project aims to reduce hospital visits and empower families.
Children with complex chronic or palliative care needs face frequent hospitalizations and fragmented care. HOME-C2P2 introduces a shared, personalised care model supported by home-monitoring devices and a digital coordination platform.
HOME-C2P2 objectives reflect a strong commitment to personalised prevention, participatory design, and digital transformation in paediatric palliative and complex care.
