HOME-C2P2 presented at the IX PEDPAL

In April 2026, the HOME-C2P2 team presented preliminary findings at the IX Congreso Nacional de la Sociedad Española de Cuidados Paliativos Pediátricos (PEDPAL), held in Tenerife on 9–10 April.
The poster, led by Silvia Ricart Campos, Eddy Carolina Pedraza-Salcedo and colleagues from Hospital Sant Joan de Déu Barcelona and Value for Health CoLAB presented early results from the metro-mapping co-creation sessions carried out in the Spanish site.
The work focuses on mapping home monitoring and follow-up models for children with complex chronic conditions, integrating the perspectives of patients (aged 12–21), caregivers, and healthcare teams across primary and hospital care settings.
Preliminary findings highlight key themes including pre-discharge training, coordination and communication channels between care levels, trust-building, periodic re-training, and transition to adult services.
